Surgery-to-Home Support

Your recovery can begin before you even arrive home.

Services may include pre-op Doctor visit, transportation from your surgical facility, assistance getting safely into your home, helping you get comfortably settled, reviewing your discharge plan with you, preparing your immediate recovery area, hydration and meal assistance, and other agreed-upon recovery support.

  • Help with pre-surgery preparation and transportation to appointments.

  • Our team have experience working with patients who are recovering from heart surgery, knee and hip replacement and other surgeries.

  • We provide services in the home, hospital, senior living center or rehab facility, when patient is at risk for falls, requires care beyond facility nursing, or when family members need help.

    24/7 sitter services to help with walking, transferring or other physical needs.

  • Help with daily living activities.

  • Medication reminders, observation and vital signs monitoring.

  • Help with meals and household tasks.

  • Coordinate care with doctors and other health care professionals.

  • Available for transportation and to go to doctor’s appointments and errands.
